The Grange: Sovereign Care North EastThe Grange provides long term care for adults with recovering mental ill health. It is a 17 bedded unit with all single bedrooms. There is a large lounge and separate dinning room . It is located in a quiet village in County Durham, but still only a short distance from major towns and cities.

Care

The Grange provides specialist care for people suffering from mental ill health problems. The care is geared towards retaining or developing skills and encouraging community based access. Each resident has a keyworker allocated. This member of staff takes responsibility for the co-ordination of the care package for that individual

Principles

The principles underpinning care at The Grange is based upon personal planning and person centred practices. Each person is seen as an individual and as such the service provided is very much individually focused.

All care packages are reviewed internally and each person has their own personal planning meeting in which they are encouraged to take a central part.

Personal planning is used to gain insight into each person's needs and requirements and from this, the care and support is designed to meet the assessed needs of each individual person.
